Cedars Sinai Marina del Rey Hospital Alert: COVID-19 (CORONAVIRUS) information for patients and visitors LEARN MORE >>

What records do I need to carry along during my first appointment?

For your first appointment, you will need to provide our medical professionals with a series of documents, such as your insurance card and a copy of your current prescription.

If this is your first appointment with a medical professional at Marina del Rey Hospital, you will be asked to provide the healthcare specialist with a series of documents and records, such as:

  • your insurance card
  • your driver’s license or any photo identity card
  • a copy of your current prescriptions
  • laboratory reports or X-ray films
  • MRI or CT scan reports which were taken recently

They are necessary to help the medical professional assess your medical history and your current health so that they can provide you with appropriate healthcare and advice. By granting your doctor access to these documents and records, they will be able to order the suitable tests and exams to evaluate your health, as well as to subsequently assign you a correct and accurate diagnosis and prescribe you the most effective treatment for your condition.
